  1. The requester submits a bounded action with an explicit principal, scope, and intended outcome.
  2. Current, decision-relevant evidence is reviewed before an approval request is made.
  3. A human approver authorizes only the bound action, and that approval expires.
  4. Execution may create an external effect, but the execution response is not proof of outcome.
  5. An effect receipt and reconciliation establish the result. A mismatch reopens evidence review.
